Overview of Available Benefits:

Group Major Medical Insurance: • Available through BlueCross BlueShield of Tennessee • Includes Vision and Prescription Card Rider • RESOURCETEK contributes a portion toward all employee only premiums • Employees may participate on the 1st day of the month following a 90-day waiting period	Voluntary Group Dental Insurance: • Intellident Voluntary Dental “Prime Plan” – underwritten by Companion Life a partner of Group Insurance Services Inc. a wholly owned subsidiary of BlueCross BlueShield of Tennessee. • Employees may participate on the 1st day of the month following a 30-day waiting period.
Group Life With AD&D: • Available through BlueCross BlueShield • $25,000 guarantee issue (for employee only) • RESOURCETEK contributes 100% of the employee only premium • Coverage effective on the 1st day of the month following a 6-month waiting period	Flexible Spending Account: • Administered by Tailored Business • Employees may participate immediately upon hire
Group Short Term Disability Insurance: • Available through BlueCross BlueShield • 60% of weekly income • RESOURCETEK contributes 100% of the employee only premium	401(k) & Roth 401(k) Plans: • Administered by Dyatech • Employees may participate after six months of service
